Membership Application

Membership Application Procedure

A membership application for voting or non-voting membership must be submitted to IFSAC Administration and approved by the Certificate Assembly prior to the entity being eligible to apply for accreditation. For more information about membership and evidence of empowerment refer to Certificate Assembly Bylaw Article 11.5.

Applications for Certificate Assembly membership are voted on twice per year either at the spring meeting or fall meeting. Applications for membership, including the appropriate evidence of empowerment, should be received by IFSAC Administration at least 120 days prior to the next scheduled regular meeting of the Certificate Assembly to be placed on the agenda for that meeting. Applications with appropriate evidence of empowerment received 120 days or less prior to the next regular meeting may be placed on the agenda of the following meeting. Accreditation/Reaccreditation Application

This application is used for initial and reaccreditation site visits and must be accompanied by the accreditation criteria worksheets and supporting documents. Administrative Review Application

Accredited entities use this application to add levels of certification to accreditation between site visits. Also refer to the Administrative Review Policy for additional information.
