Applications & Fees - Certificate Assembly

A membership application for voting or non-voting membership must be submitted to IFSAC Administration and approved by the Certificate Assembly prior to the entity being eligible to apply for accreditation. For more information about membership and evidence of empowerment refer to Certificate Assembly Bylaw Article 11.4. Applications for Certificate Assembly membership are voted on once per year at the Certificate Assembly annual meeting held each April. Applications for membership and evidence of empowerment should be received by IFSAC Administration at least 60 days prior to the Certificate Assembly annual meeting.
This application is used for initial and reaccreditation site visits and must be accompanied by the entity self-study and supporting documents. For a complete list of required application and self-study documents as well as timelines, refer to the Certificate Assembly Accreditation Procedures.
Accredited entities use this application to add levels of certification to accreditation between site visits. Also refer to the Administrative Review Policy for additional information.
