- Non-shaded areas of the Institution Self-Study and Evaluator Checklist are to be completed by the program applying for accreditation or reaccreditation and the document submitted to IFSAC Administration at least 90 work days prior to the anticipated site visit.
Degree Program Final Action Report Form (utilized by evaluation team members when deficiencies are identified)
