Application for Accreditation

• An application for membership must be submitted and approved before an entity is eligible to apply for accreditation. For more information on rules of the Degree Assembly read Bylaw Articles 20 through 21 and the Degree Assembly membership and accreditation process.

Institution Self-Study and Evaluator Checklist Amended April 2025

• Non-shaded areas of the Institution Self-Study and Evaluator Checklist are to be completed by the program applying for accreditation or reaccreditation and the document submitted to IFSAC Administration at least 90 work days prior to the anticipated site visit.

Degree Program Final Action Report Form (utilized by evaluation team members when deficiencies are identified)
